Candidate Information

Information on this page is intended for individuals who are applying for Probationary Membership and Commissioning for Ministry or Full Membership and ordination as a Deacon or Elder in the Desert Southwest Annual Conference. Please mail the completed application to Stewart Lewis, Registrar, 5540 E. Main St., Mesa, AZ 85205.

All necessary forms are available for download.

Deadlines
It is your responsibility to learn the deadlines for applications and paper submissions. This information will be published on this on the Important Dates page of this web site, and in conference newsletters. You may always contact the registrar (Stewart Lewis) to confirm these deadlines. Please be aware that deadlines do change from year to year because of many different factors. Do not assume that last year's deadlines will apply this year.

Requirements are that the Candidate, the candidate's home church and the BOM will each pay 1/3 of the costs.


Formatting of Papers 
Since Board members will most often read your papers on line, we require the papers to be submitted in a format that facilitates their placement on the BOOM Secure Web Site.

  1. Our preferred format is Microsoft Word. (Email Stewart Lewis if you have questions about file compatibility.) Papers will be accepted in Microsoft Word (*.doc), Rich Text (*.rtf), and Adobe Acrobat (*.pdf) formats. Most word processing programs will allow you to save your work in one or more of these formats.
  2. Please submit each major section of documents as a separate file. In other words, you should have a file for your doctrinal exam, a file for your pastoral care exam, a file for your exegesis, a file for your sermon, a file for your teaching plan, etc.
  3. Please include your name, document title, and date at the beginning of each paper, i.e., “John W. Candidate, Doctrine Examination, November 20, 2003.” This should be the first line of text in your papers.
  4. Please use standard fonts and page sizes. Endnotes are preferable to footnotes.
  5. Always keep a hard copy, and an electronic copy, of your submitted papers. BOOM is not responsible for electronic transmission errors or files lost in the process of dissemination. Keep a copy in the event files are lost or damaged.
